Job Summary:
The Prevention Coordinator is responsible for being a member of a team tasked with facilitating a combination of youth service programs. The Coordinator will provide substance use prevention education services to youth, families, and community members and work with schools and community organizations to facilitate these services. Using evidence-based strategies and programs, the Coordinator will ensure that community stakeholders have comprehensive substance use education and other prevention services available, thereby assisting in developing healthy communities.
Work Location: Elevate Office 1426 N. 5th Street, Sheboygan, WI
Hours: Full Time, Hourly/Non-Exempt, 8:00 am – 4:00 pm (flexibility required)

- Coordinate the implementing and expanding of curriculum and programs applicable to middle and high school levels.
- Co-facilitate the Strategic Prevention Framework process with community action teams to assess needs, mobilize and build capacity, develop a comprehensive strategic plan, implement evidence-based strategies, and measure the effort's impact.
- Implement group education/behavior change workshops using the evidence-based curricula.
- Conduct youth assessments and one-to-one early intervention sessions.
- Assist with expanding other targeting programming into new service areas.
- Aid in collecting, maintaining, and analyzing data for evaluation, fund development, and marketing purposes.
- Support other Youth Services programming by implementing prevention education at the middle and high school levels.
- Represent the agency at all appropriate community meetings and events.
- As appropriate, assist with implementing special initiatives, including, but not limited to, advocacy, parent education, and social marketing campaigns in our community and other coalition activities.
- Expand research-based ATODA programs in schools/communities served.
- Participate in agency staff/committee or program meetings and attend all designated agency functions.
- Assure thorough documentation and accurate, timely data entry into the agency’s electronic data capture system.
- Provide leadership and/or support in implementing alcohol and drug-free social or community service activities.
- Provide support to other Youth Services programs, as needed.
- Assist in identifying public policy initiatives to advance our mission.
- Assist in implementing other environmental strategies in the community.
- Prepare program activity reports for evaluation, fund development, and marketing purposes.
- Participate in agency events.
- Other duties as assigned.
- Minimum of a bachelor’s degree in public health, social work, or other related fields is required.
- Experience organizing, training, and supporting adults and youth to engage in public action.
- Knowledge of outcome-based evaluation, prevention theory, substance use, and mental health.
- Excellent verbal and written communication skills.
- Honed organizational skills and ability to multitask.
- Public speaking and presentation skills.
- Relationship-building skills and experience.
- Proficiency in Microsoft Office Suite and Google Applications

- Pre-employment criminal background check, employment background check, driver’s license check and drug screening required.
- This position may require travel throughout counties served by Elevate, Inc. (primarily Sheboygan County).
- Must have a reliable vehicle and valid vehicle insurance.
- This position requires working in public school settings and may require occasional early morning starts to accommodate school schedules.
- This position serves community events, which may occasionally require working weekend and evening hours.
- Must be willing to have training in the administration of Naloxone within the first six months of hire and be willing to administer Naloxone in the case of a suspected opioid overdose.
