Are you passionate about helping individuals overcome substance use challenges while making a positive impact in your community? Elevate is seeking a compassionate, organized, and motivated Case Manager to join our Treatment Alternatives and Diversion (TAD) Program.
In this role, you will work with individuals involved in the criminal justice system who are living with substance use disorders. Through intensive case management, collaboration with treatment providers, attorneys, and the courts, you will help participants successfully complete diversion programming, reduce recidivism, and build healthier futures.

Pay:
$24.00–$26.00 per hour

Hiring Bonus Details
  • 50% ($600) is paid on your second paycheck after hire.
  • 50% ($600) is paid after successfully completing six months of employment, provided you remain in good standing with no disciplinary actions.
  • Employees who voluntarily leave employment before completing one year must repay a prorated portion of the second installment only. The initial payment is not subject to repayment.
Essential Responsibilities
As a Case Manager, you will:
  • Coordinate referrals with the Washington County District Attorney's Office and defense attorneys to determine eligibility and facilitate timely program enrollment.
  • Provide intensive case management for individuals participating in diversion programming.
  • Assist clients in accessing substance use treatment, mental health services, housing, employment, education, transportation, and other community resources.
  • Develop individualized diversion and treatment plans in collaboration with clients and multidisciplinary treatment teams.
  • Monitor client progress, reinforce recovery skills, and support long-term sobriety and successful program completion.
  • Provide coaching on relapse prevention, coping skills, craving management, refusal skills, and recovery supports.
  • Collect urine drug screens and preliminary breath tests, accurately document results, and communicate findings with treatment providers and other authorized partners.
  • Collaborate with the Wisconsin Department of Corrections to coordinate treatment goals and supervision requirements.
  • Maintain timely, accurate, and confidential client documentation, including assessments, progress notes, testing results, collateral contacts, and discharge summaries.
  • Enter required client information into agency and grant reporting databases.
  • Participate in staffing meetings, court hearings, case reviews, and graduation proceedings.
  • Maintain required professional licensure, certifications, and continuing education.
Qualifications
Required
  • Bachelor's degree in Social Work, Psychology, Criminal Justice, Health Education, Human Services, or a closely related field.
  • Licensed Substance Abuse Counselor-In Training (SAC-IT) required.
  • Knowledge of confidentiality laws, including HIPAA and 42 CFR.
  • Experience providing case management, counseling, or intervention services to individuals or families.
  • Strong verbal, written, and interpersonal communication skills.
  • Proficiency with Microsoft Office and Google Workspace.
  • Complete Naloxone administration training within six months of hire and administer Naloxone when appropriate during suspected opioid overdose emergencies.
Preferred
  • Experience working with individuals experiencing substance use disorders.
  • Experience in criminal justice, diversion programming, corrections, or behavioral health.
  • Knowledge of community resources and recovery-oriented systems of care.
